A questionnaire was sent to 600 Dutch community-living older adults aged 75 years or older and living in the municipality of Emmen. Community-living older adults reported the need for: accessibility of medical records on the computer (AMR), receiving information and advice (RIA), working on your own health (WOH), confidentiality and privacy (CP), and accessibility and user-friendliness (AUF). Limited need was found for online communication with the healthcare professional (CHP). Older adults who had no intention to use a PHR, nevertheless reported to have the need for AMR and CP. Older adults with high functional health literacy and high computer literacy were more likely to have the intention to use a PHR. Efforts to increase the user-rates of the PHR among older adults should address the needs that are identified by this study.
